Lifetime Family Medicine is a small family medicine practice located in Haymarket, Virginia.  It was opened in the fall of 2006 by Dr. Theresa Shupe, formerly practicing in nearby Manassas, to fulfill a life-long dream of an independent  family-centered medical practice.  Dr. Jocelyn Serrano joined the staff of Lifetime Family Medicine in the fall of 2007.

Lifetime Family Medicine has been chosen by the American Academy of Family Physicians (AAFP) as one of 36 practices in the United States to participate in the TransforMED program.  This National Demonstration Project is a groundbreaking study, funded by the AAFP to implement a new "patient-centered" model of health care. This model is based on the concept of a relation-centered personal medical home.  The eight core elements are:

1. Patient-centered care
2. Whole-person orientation
3. Team approach to care
4. Elimination of barrieres to access; open access by patients
5. Advanced information systems, including electronic health records
6. Redesigned, more functional offices
7. Focus on quality and safety
8. Sustainable reimbursement
